Forms allow you to collect data from your customers. Forms & Landing Pages at EmailsAndSurveys are full featured forms and can have multiple pages, have creative effects and be part of automated Workflows. You can create your own web form that can be hosted right here at EmailsAndSurveys, on your site, or maybe a partner site as we provide you with the URL and code to do so.. At EmailsAndSurveys, they have many applications. You can attach them to a Survey or Assessment to collect more information based on an answer. All customers should put a form up to collect contacts for your Email and SMS campaigns.

Step 1
To Create a form, navigate to My Desktop > My Pages Design > Forms Design or just click My Desktop > My Forms Design.
To edit or delete a form, click on your Form and you can edit it, delete it or get its public link. Forms are created with the same editor our our Email Editor so you have full control on branding your Form. You may want to watch the video on How to Use the Editor at this time.
Step 2
Give your Form or Landing Page a name and indicated if you want to be notified upon form submission and if this is a one page or multi-page form.

If you click the + icon you will be given the opportunity to add a new Survey Page or a Landing or Content Page to be placed in your survey. To delete one of these pages, use the trashcan on the right
On the Editor Palette, you will see a wheel cog that has Page defaults for background, text, border, margin and padding. The available forms blocks will be available to you in block icon and the thumbs up icon has Social Media options for your Form. You will have full control of the Form to brand the look in feel according to your web presence.

Step 4
The next Step is to define the header and footer for your Form. Pick a style that you like and set your image and any text that may be required. You will be taken to My Drive where all your images should be uploaded. You will be given the opportunity to change image settings to fit correctly in your Form. If not just edit the image and resize it in My Drive. There is an attacked article on how to use My Drive.
And your footer. EmailsAndSurveys always encourages are customers to put links and Call To Action to your Social Media Pages.
Step 5
Please take time and look at all our form input controls, we have included Survey input controls if you you like like to corporate a survey, mini-poll or just see how people feel. There is a pre-built Lead Capture Form. In this example we are asking for Name, Email Address and Phone Number.
Step 6
To make your Survey fun and engaging, EmailsAndSurveys allows you to have different page effects for how Questions and Pages are rendered. Take a look at these effects and see if you would like to use one of them by click the block to set Block Settings.
You also have the option to set images and with your questions and set how the question should transition.

On Page 2, we are going to ask them how they feel about a a subject

Once you form is complete, click Publish
Step 9
Once you form is published, you have a number of options where it can be hosted. You can send in an email or SMS, host here at EmailsAndSurveys or post on your website.
View Your Data
From the main forms page, click Data Browser and then click the pie icon to view your data which can be downloaded and soon be associated to a Group
